Your Will should utilise your current full legal name. In the alternate name section, you should only include previous names that you still hold assets in. If you hold assets in multiple previous names, you can add them into the Alternate last name box as follows.
Including alternate names, such as maiden names or previously used names, ensures that your executor can locate and distribute assets held under those names. This is particularly important if you have legal documents or accounts registered under different names.
If your middle name appears on some legal documents but not others, include it in the alternate name section to cover all scenarios. For example, enter "Mitchell Middle Name Cunningham" under alternate names if this variation exists.

Alternate First Name: Sally
Alternate Middle Name: Jessica
Alternate Last Name: Parker, Sally Jessica Smith, Sally Anne Smith-Parker
Additionally, if your name has changed due to marriage or other legal reasons, ensure that both your current name and any previous names are included in the will. This ensures that assets held under a previous name are not overlooked.
